Use this form to request financial help with rent if you have experienced a sudden loss of income.

Who should use this form

  • Grand Island resident with a current lease
  • Experienced involuntary income loss in the past 90 days
  • Household income at or below 80% of Area Median Income

What you need before you start

You cannot save and return. Gather all documents before you begin.
  • Government-issued photo ID
  • Current lease agreement
  • Proof of income loss (termination letter, pay stubs, unemployment notice)
  • Landlord name, phone, and mailing address

After you submit

You will receive a confirmation email within 24 hours. A housing specialist reviews your application and contacts you within 10 business days. If approved, funds are paid directly to your landlord.
